Instructions: Start by filling in the PAY PERIOD dates that always start on SUNDAY and end on SATURDAY. The PAY PERIOD is for 1 week. Fill in the artist name, your name, address and bus name. Then fill in the dates you actually DROVE within the pay period. Fill in the number of days and overdrives and the rate you are to be paid.
